Wikipedia and copyright
Hello Carroolll, and welcome to Wikipedia. Mouth
Multiple myeloma of the mouth , can appear similar to a dental abscess, gingivitis, periodontitis, or other gingival enlargement. The initial presentations may involve pain, paresthesia of the inferior alveolar and mental nerves, swelling, expansion of the jaw, numbness, tooth mobility and radiolucency.[1] Furthermore, some extraosseous lesions might mimic gingival enlargements or epulides. The lesions might extend to other sites outside the skeleton such as skin, lymph nodes and viscera. Petechial hemorrhages of the skin and oral mucosa can be seen if the platelet production has been affected.

Pre-lymphoma treatment assessment (pre-chemotherapy cycles, monoclonal antibodies and pre-radiotherapy)
•Before starting a lymphoma treatment, a dental assessment is recommended to make sure that patients are dentally fit.
• The patients should receive periodontal treatment, restorative treatment or adjustment of ill-fitting dentures if necessary.
• Dental extraction of teeth with poor prognosis at least 10–14 days prior to chemotherapy cycles and monoclonal antibodies is recommended. Otherwise, it is recommended to wait until the next cycle of chemotherapy and manage any dental infection with antibiotics with/without pulp extirpation.
• Dental care providers should avoid dental extraction for patients with platelet counts below 50 x 109/L, and no regional block injection for patients with platelet counts below 30 x 109/L. Avoid giving conscious sedation for patients with hemoglobin below 10g/dl.
• Prior to dental procedures, antibiotic prophylaxis for patients with neutrophil counts below 2 x 109/L is recommended to reduce the risk of septicaemia (granulocyte colony-stimulating factor) • Topical anti-fungal prophylaxis could be prescribed for herpes simplex viral infection
• Advice the patients to take sips of water, use sugar-free chewing gum and saliva substitute.
• Patients should consider soft diet instead of rough and spicy food, acidic fruit and salt.
• Cessation of smoking and alcohol is advised
• Oral hygiene instructions to include using a soft toothbrush with alcohol-free mouthrinse.
• All dental treatment should be planned in liaison with patient’s haemato-oncologist.
During chemotherapy cycles or radiotherapy
• Only urgent dental care should be given at this stage, managed with antibiotics and analgesics with/without pulp extirpation.
• If dental extraction is necessary, it should be carried out in hospital setting in liaison as patients might need blood transfusion at this stage.
• Patients should receive oral hygiene instructions and maintain good oral hygiene.
• Avoid mucositis by using soft toothbrushes or sponge toothbrush with alcohol-free mouthrinse. Topical gel could be considered to control pain.
• For the treatment of fungal and viral infections, prescribe anti-fungal or anti-viral medication.[2]"
