Urethra

integrity of bladder wall

In medical diagrams, the prostate is located outside and below the bladder, with the urethra passing through it. Thus it would seem that any transurethral procedure, operating from within the bladder to remove tissue from the prostate, would have to cut through the bladder wall to reach the prostate. Why this does not happen is not well explained. Does a lobe of the prostate fuse with the bladder wall and protrude into the bladder? If this is what is removed, how does that help with restriction of the urethra where it passes through the prostate below the bladder? Explanation of the "ball-valve" effect might be warranted here.
