Incorrect information

I'm not sure who created this article, but it contained a fair amount of misinformation regarding the etiologies of pneumobilia (pancreatitis??, pneumobilia = fistula??), and much of the organization and information seems to be based on a single paper on choledochoenteric fistula. In fact, most of this article is actually about biliary enteric fistula and not about pneumobilia at all. I move that all of this be removed, and I will try to transfer relevant information to a choledochoenteric fistula page (if it exists).
