WikiProject Medical Student Edit

Hello everyone, I am a 4th year medical student that is going into internal medicine. My medical school is part of an initiative called "WikiProject Medicine" in which a student works with the community on an article of their choosing to improve it with the latest up to date knowledge and citations. I have a preliminary interest in nephrology and thus settled on this topic. This article was identified by WikiProject medicine as having start-class quality with mid-importance and is supported by the nephrology task force. Please find below the list of sections I will be looking at and how I hope to improve upon or add them into the article.

-Signs and symptoms: This sections has a lot of great information but I believe it needs additional citations that I would like to add in. I also see some information that I believe would better fit a "Pathophysiology" section. There is also a lot of information regarding imaging findings that could be moved to a separate "Diagnosis" section for improved organization. I think this section would benefit concentrating on just patient presentation with history/physical exam findings. I will include information regarding what patients with renal fusion are more predisposed to such as vesicoureteral reflux and increased incidence of UTI's/kidney stones.

-Diagnosis: I plan to create this sections and move some of the imaging findings listed currently in the signs and symptoms section to here as well as include more detailed and up to date information with the appropriate citations.

-Pathophysiology: I plan to create this section and move some of the information included in the signs and symptoms section to this one as well as well as expand upon it.

-Treatment: I plan on creating this section and will look for sources that show consensus on management of patients with horseshoe kidneys and add that information to this section. From my preliminary research, it appears that most of the population with renal fusion are asymptomatic and have excellent prognosis though.

-Epidemiology: I plan on creating this section and updating it with information regarding prevalence in general population as well as in special patient populations that have genetic diseases that predispose them to this condition (e.g. Edward syndrome, Turner syndrome, etc.). The male preponderance would also be moved to this section.

-Notable cases: I don't plan on changing this section much. I will mostly look for better sources for the information already listed here as well as any additional public figures.

WikiProject Medicine Student Review

Overall, I thought that the article was well written. It is much better organized and definitely met your goals. I think the vocabulary is appropriate for most people. Its primary weakness is some of the references, which are not ideal (like that StatPearls one). However, there just does not seem to be that much good literature on horseshoe kidneys so I think it's best to just use those references anyway. I would also create wikilinks for: renal artery, thalidomide, ethanol, ACE inhibitors, cocaine, gentamycin, corticosteroids, NSAIDS, glucose and colon.

Pathophysiology: I think this should be after signs and symptoms based on the wiki manual of style. Otherwise, it is a solid summary. I think it would also be appropriate to have a separate section called etiology for the second paragraph, which is really focused on possible causes.

Signs and symptoms:I thought this was well written and succinct. The only thing that I did not like was the #9 reference for kidney tumors and it is not needed since you already have another reference for that sentence.

Diagnosis: The information discussed is complete, relevant and succinct. The move from signs and symptoms is definitely appropriate. Everything is cited.

Treatment: I think that it would be good to touch on percutaneous nephrolithotomy (or at least mention it and link it to the wiki article) when you are talking about large kidney stone removal thru a "minimally invasive procedure." Otherwise I thought that the section is solid.

Epidemiology: Solid and succinct.

Notable cases: The reference for Robert Rowan does not seem to be about Robert Rowan. I think that this person should be removed unless a proper reference can be found.

Thank you for your feedback and I 100% agree with your thoughts. I have incorporated all that you have suggested. One quick note on the resources is that, as you say, it was rather difficult to find good review articles or textbooks that talked about horseshoe kidneys in any reasonable capacity. The statpearl source was, unfortunately, one of the best I could find. However, I did find the primary articles that it referenced and made sure the information matched up with it. In some sections of the article, I also referenced some of the primary research in addition the review article. This was for the reader should they be interested in looking at it.
