Indication: For relief of symptoms in various depressive syndromes, especially endogenous depression.

Pharmacology: Desipramine, a tertiary amine tricyclic antidepressant, is structurally related to both the skeletal muscle relaxant cyclobenzaprine and the thioxanthene antipsychotics such as thiothixene. Desipramine is used to treat depression, pain of neuropathic origin, attention_deficit hyperactivity disorder, functional enuresis in children, panic and phobic disorder, and to manage some eating disorders. Desipramine inhibits the re-uptake of noradrenaline at the noradrenergic nerve endings and the re-uptake of serotonin (5-hydroxy tryptamine) at the serotoninergic nerve endings in the central nervous system. These two effects are considered to be the likely base of the antidepressant effect of Desipramine. The drug also has a strong anticholinergic effect and serves as an antagonist on a1 and H1 receptors.

Mechanism of Action: Desipramine is a tricyclic antidepressant that selectively blocks reuptake of norepinephrine (noradrenaline) from the neural synapse. It also appears to impair serotonin transport. Desipramine also possesses minor anticholinergic activity, through its affinity to muscarinic receptors. Evidence also suggests that Desiprmaine binds to and down regulates histamine and beta adrenergic receptors. Tricyclic drugs are believed to act by restoring normal levels of neurotransmitters by blocking the re-uptake of these substances from the synapse in the central nervous system.

Absorption: Desipramine hydrochloride is rapidly and almost completely absorbed from the gastrointestinal tract. 90.5% (range 73-92%) of desipramine binds to plasma proteins.

Toxicity: Male mice: LD50 = 290 mg/kg, female rats: LD50 = 320 mg/kg

Protein Binding: 15%

Biotransformation: Desipramine is extensively metabolized in the liver and is cleared mainly by 2-hydroxylation and glucuronidation, whereas 10-hydroxylation is of minor importance. The 2-hydroxylation metabolic pathway of desipramine is under genetic control.


Desipramine and ADD & ADHD

I do take desipramine for ADD and it works very well. :)

The article is currently way off. Desipramine probably *does* do good stuff for the NE-related subtypes of ADD. Just like reboxetin, atomoxetin, etc.. Stating it is better than ritalin, without a source, is asking for trouble, though. Don't put it back in without a cite, as it might appear very POV otherwise. desipramine for years

I had a hysterectomy and was forced to take premarin...I was allergic to it...the military insisted that I was depressed and that is why I was having all sorts of wierd reactions after the surgery. they never bothered to check if I was allergic to premarin. I went to a civilian dr and she ran a blood test and and discovered that and told me to get off. I was not sleeping or eating and had tons of other side affects happening from premarin. I am allergic to equine..horses..dahhh...back then women who had hysterectomies were forced to take hrt...harmone replacement therapy. I wasnt in the military, my husband was. I had 5 kids and was happy and maybe a little sad. I agreed to see a shrink who prescribed desipramine so I could at least sleep and eat ..it however, didnt help the mood at all. later I was tried on the tape and was allergic to the adhesive. then I was tried on estrodiol and within three days it worked and my moods were great.

as a result, I stayed on desipramine and estrodiol and occasionally zanax as desipramine caused some anxiety. I started desipramine in 1989 until 1997 and tried to get off. I had my first seizure. I went through a lot of pain. I was jerked off the last 100mgs right in the emergency room and put on dilantin. I was allergic to dilantin and broke out in a rash. I then was tried on the new ssri's and felt awful and tried on neurontin. I thought it was all of those drugs that was making me feel awful...but not until now I realize that the reason at that time that I was feeling so bad was because of the dr's jerking me off desipramine fast to put me on dilantin. it affected my intestines. I couldnt stop my intestines from moving, and it dragged me out and I became drepressed. I then decided to put myself back on desipramine as no one could figure out what to do. I went to another shrink and he got me back on and I was ok.

later in 2006 or so I had another seizure trying to get off desipramine..I was on 120mgs....the emergency room put me on keppra and I continued to get off desipramine...I then got off all my meds..estrodiol and desipramine and keppra...everything...I had been off everything since abt september 2007....I was having problems with desipramine messing up my stomach...then....I had another seizure a month ago...I have been off desipramine abt 6 months....I figured that the desipramine was out of my body...and I shouldnt be having any more seizures...

I never had seizures in my life before until 1997 when I was trying to get off only....and each time trying to get off I had a seizure....while on desipramine I didnt have any seizures....I was fine...so...I was put back on keppra alone....Im on 1500mgs keppra...its not fun getting on this stuff....it is causing me depression...Im hoping it goes away and is just an initial side affect....it does say that it can cause mood swings....as one of the regular side affects....but some of the bad side affects are...depression and other things...anything that affects the brain can do wierd things....

I dont want to get back on desipramine....its not good to be on something like that...I am old...56 yrs old..and could stay on it the rest of my life...but it was causing me gut problems and not actually working for depression at all. I went through depression twice on it. I would like to know if anyone else has had seizures when getting off desipramine...

I do have to say that I had a problem with being a little shy and never went to college until I actually started taking desipramine...I made good grades....I was able to concentrate better on it...I probably was a little add...something like that...I did have a history of major depression and anxiety disorder but wasnt taking desipramine for that...but stayed on it in case....Im sure it has become a part of my body...and its tough getting off for the body to adjust...I just am tired of getting seizures..I have had 3 of them now and as a result of trying to get off and being off for 6 months...

I heard that it can cause seizures going on it....however that didnt happen to me...just getting off...mind you I have gotten off all that and zanax for an acute case of IBS...and never had a seizure while on desipramine....who knows...has anyone had any kind of experience like this....seizures when trying to get off....right now I dont like taking keppra for seizures...I hate the side affects of taking something new...the keppra also at higher doses causes head aches...which I do not have.. —Preceding unsigned comment added by Vickiec51 (talkcontribs) 04:25, 25 February 2008 (UTC)[reply]
