Cleanup, ce, references

I cleaned up the article structure (things like WP:MSH), and added citations. I see that most of the article content was a direct copyvio from the ABPN website: I hope that will be avoided in the future. I reworded to the extent I could to avoid plagiarism. I can see what you mean but I think the real problem is the history section is so scant. I've tried to clarify the mention of Freeman but he does seem to merit inclusion as a notable fact in the history, partly because of how controversial he and his practices were. Board Certified/Board Eligible

How common is board certification, as opposed to board eligibility? The various American Boards state quite clearly that "Board Eligible" status last only for the short time between being approved to sit a particular exam and the date of that exam, but they said it so often that one might suspect that in practice people who had completed residency training said they were Board Eligible without going on to take or pass the exam.
