Inserting text

In Signs and Symptoms you added words (shown here in bold) to make the text "A medical sign is an objective observable indication of a disease, injury, or abnormal physiological state that may be detected during a physical examination, examining the patient history, or diagnostic procedure." The text as it was, is immediately followed by a reference. My advice is that you look at that reference to see of it supports your revised description of a medical sign. Be not afraid

Some/many of the medical/health articles are a horrible jumble because people have been adding to them over years. (Clicking on View history provides a chronoligical list of edits; then clicking on prev show a side-by-side of that edit, while clicking on the date shows how the article looked after that edit.) See Help:Referencing for beginners for general info on creating references. There is a shortcut for medical journal refs. If a search at PubMed locates refs you want to add, those will have PMID numbers. Open up https://tools.wmflabs.org/citation-template-filling/cgi-bin/index.cgi then enter the PMID number and it generates a reference. Just remember to add the ref and /ref at the beginning and the end when you insert the ref into the text. The ref system assigns superscripted numbers and adds the ref to References, at the bottom.
