Prognosis data

Prognosis data for various diseases is very difficult to find because there is no centralized place to collect the data. I think a comprehensive list (with references) of what prognostic information is available would be very useful. Particularly for health care workers that need to know the data for many different kinds of diseases. Wikipedia is the perfect way to do this because everyone can contribute.
