Price

£4015 for pack of 84! £47.80 per capsule. Most expensive capsule purchased by a children's hospital in the London area. There should perhaps be a mention of the high cost of the drug, can somebody explain why the cost is so high? 82.211.83.9 (talk) 14:17, 22 May 2009 (UTC)[reply]

Developing, manufacturing and distributing a drug that brings benefit for only a few hundred patients world wide explains the high price. Especially developing a drug takes years and pharmaceutical companies have to follow stict protocols imposed by health authorities and requiring costly clinical studies. Once approved and authorized for sale, the money gained serves to pay back the money previously spent during the years of development. 212.27.185.253 (talk) 08:27, 23 April 2010 (UTC)[reply]
At our children's hospice we have provided suport for a small number of children who have been prescribed this medication. A number of them have had diarrhoea that has responded reasonably well to a lactose free diet. Mechanism of Action

As (correctly) stated under “Chemistry”, “Miglustat is an iminosugar” of glucose, i.e., the O (oxygen) in the sugar ring is replaced by an N (nitrogen). With many enzymes that use glucose (not only glucocerebrosidase), it binds the active side binding glucose, but the enzyme then cannot act, resulting in a reversible inhibition. Other enzyme affected u alpha-galactosidases A (α-GalA), which causes the known gastrointestinal side effects (diarrhea, …).
